Is a special Greek island-entry permit equivalent to a Schengen visa?

No. Arrangements for designated islands have their own territorial limits, stay allowances and advance application requirements. Check the current participating islands and entry points, and do not assume the permit allows travel to mainland Greece or elsewhere in Schengen.
